Well said, @TYoungstein. Those of us with safety concerns around the role of PAs - now, finally, corroborated by Leng - were smeared as an irrational, toxic rabble. I think Chris Whitty owes doctors an apology. As do @RCPhysicians, @JimBethell et al.

Whilst reading the Leng review & considering the @RCPhysicians response (irony itself) It’s worth remembering this exchange from the exPRCP & current CMO It seems the passionate minority whose rationality was “way down the list..beyond being swayed” - were actually correct?

As an NHS doctor I resent this deeply Why are my mandatory GMC fees being used to pay for private health care for GMC employees?! I don’t even have private healthcare myself! **£642,000 spent on 1066 staff** ‼️ I do NOT give consent for my fees to be used in this way! 🤬

In 2023, @gmcuk awarded a four-year £2.8 million contract for private medical insurance (£700k per year) 💰 The tender documents show the actual spend on private medical insurance in 2022-23 was £642k for 1066 staff This is equivalent to 3690 x FY1 fees or 2006 x PA fees

So the PA examined and took the history of a patient. And the tribunal determined that the doctor failed in their duties for not examining the patient or taking a history themselves and in addition to the PA. What exactly is the purpose of a PA in this medico-legal context?

Absolutely. This is a significant part of the problem. Another significant part of the problem is people advocating that less qualified and unregulated “associates” with less training, and both lower training and entry standards, are suitable alternatives to GPs. Who knew?! 🤷🏻‍♂️

“Physician associates” with no medical training ➡️ REPLACING doctors❗️ “Nursing assistants” with no nursing training ➡️ REPLACING nurses❗️ “Pharmacist technicians” with no pharmacy training ➡️ REPLACING pharmacists❗️ I’ve an idea - let’s try “Politician associates” next!

Patients aren’t asking for much. Sad to say if you look up you get asked for updates, chasing investigations and ‘how long is the wait’, and even worse; blankets, food, help going to the toilet, medications. You try and do what you can but you have to prioritise the sickest.
