My burn out cycle: 1. Feel good, so start doing something consistently. 2. Able to handle it, so add another thing. 3. Able to handle that thing too, so add another. 4. Repeat until burn out. 5. Be shocked that it happened, wait until feel good again and restart the cycle.

Focus sprints of 2 hours each, 15 minute planning for the goal of the sprint, no distractions, work to achieve the goal of that sprint. It takes some time to choose small enough goals. 30 minute breaks in between. Real breaks, outside, NO work or comms.
