I think that there's a difference between a mistake and error, which is explained well in this table. #SoMe4Surgery #SoMe4Error
Better training & awareness of the need to support the Vaginal Apex during Hysterectomy. Failiure to support can lead to Iatrogenic Prolapse ie Vagina, cystocele, rectocele. More frequent in Generalists & Gynaecologists Then #Mesh used #SoMe4Error @SoMe_so link.springer.com/article/10.100…

Agree. Not all complications are errors and not all errors cause complications. However as I say repeatedly to my team - just coz we got lucky doesn’t make it right! 🤞🏻 #SoMe4Surgery #SoMe4Error #Q1
Surgeons usually receive support from colleagues, friends and families with the best intentions, not always optimal. You can do your best for ur patients, not always the best 4 them, but at least you can ask your institutions 4 support #some4surgery #SoMe4Error
Well put @DSoybel #SoMe4Surgery #SoMe4Error @me4_so
Errors can occur everywhere, including transitions from ICU to ward, or Xrays or wherever. Different factors: papermismatches, technological problems, work pressure, rush, lack of attention... #some4surgery #SoMe4Error
#SoMe4Surgery #SoMe4Error Our documention system is computerised, but not easy to extract the data, so another possible risk area
Great initiative @stefanie_hild #SoMe4Surgery @me4_so #some4Error
I agree, harm is part of it. Deviation from the "norm" (lex artis) is another factor.

Thankyou @MothersWith4DTs for joining the discussion. I think, essentially, Patients, don’t want to remain Patients forever. They want early, effective interventions from skilled Drs to stop deterioration A Hospital Exit door Not a Revolving door #SoMe4Error #SoMe4Surgery @me4_so
I can just imagine that many patients, may find that some or most of their patient notes are missing, after switching provider, or they can’t be accessed @MothersWith4DTs It’s important for patients to receive/request outcome letters when treated @me4_so #SoMe4Error #SoMe4Surgery
It is one who challenges makes us think. Coming to #patientsafety many hard working good doctors nit aware because many have not seen the impact. 36 years as Paediatrician two patients suffered serious harm and I was and still am devastated. Both had systems failures #SoMe4Error
Yes patients want a Dr of their choice, based on results. Patients seek specialists in complex surgery ie for #Mesh eg. the Brave & skilled Urogynaecologist & Neuro-gynaecologist @ElneilSuzy & she trains others @me4_so @swexner #SoMe4Surgery #SoMe4Error
“I would like to congratulate Sohier Elneil - NHS consultant and dedicated champion for her patients who won Health Hero of the Year. Sohier and all of the nominees epitomise the very best of our NHS.” - PM @Theresa_May at Daily Mail Health Heroes awards
@MothersWith4DTs site explains their #PatientExperience , care difficulties & emotional/physical issues after birth. This patient category for #ColorectalSurgery is poorly researched & with severe injuries @me4_so @AlaaEl_Hussuna #SoMe4Error #SoMe4Surgery
There are many patients who share their #PatientExperience in blogs & twitter feeds @mjseres’s has been used by medical schools @AlaaEl_Hussuna @0X2020 #SoMe4Surgery #SoMe4Error @SWexner @juliomayol @me4_so
Can a patient take their care into their own hands to help improve outcomes? Here is @mjseres’ story
To all HCP😷🤒👷 #problemsolving #healthcare Taking these comms errors as a case studies, each incidence should be part of a problem solving improvement project as described in my book bddy.me/2v6v1YF. Next reply ➡️
Many thanks @AlaaEl_Hussuna @SWexner @juliomayol @me4_so for considering issues in #SoMe4Surgery. We all want healthcare improvements. It results in net benefits for Patients & Drs Reducing #AvoidableHarm, Improves Patient QOLY & Audits & reduces litigation & ReOps & #SoMe4Error
This is why I asked. @NHSEngland research on Lower urinary tract fistula in the UK confirms it is commonly iatrogenic & occurs often after pelvic surgery. Post Hysterectomy accounted for 50% of all cases @me4_so #SoMe4Error #SoMe4Surgery england.nhs.uk/wp-content/upl…
Fistula/SUI often occurs after Birth Trauma following prolonged labour/CSec & Hysterectomy. #Sling #Mesh complications after these issues are often worse. Would improved gynaecological training assist to prevent, treat & detect earlier? #SoMe4Error @me4_so bsdwebstorage.blob.core.windows.net/ejournals-2218…

I may Draw fire by this question. But: Why don’t Obgyn Surgeons receive as much Surgical training as other surgeons? Many issues seem to come from this dept ie fistulas, bowel Incontinence ie after Vag. Hysterectomy/physical birth trauma tears @me4_so #SoMe4Error #SoMe4Surgery
It begs the question. Should Harmed Patients & Drs stay quiet & not voice concerns? Will that protect #PatientSafety? Should they assume harms stop happening, if they say nothing? Should they suffer, in silence, in their 1,000s for fear of offending someone @me4_so #SoMe4Error
Perhaps part of the reason, Deficits are arising, is due to failing to Research & Budget for Longterm outcomes, ie making the hospital Exit, 1 way, Rather than a Revolving Door Research needs to find the Best Treatment, not 1, surrogate outcome #SoMe4Surgery #SoMe4Error @me4_so
Agree. Theres alot of flawed research & Very few Longterm Qualitative Patient Studies to balance against it. Patient Orgs shouldnt only, contribute as participants, they should be part of Designing the Trial, PROMS & Patient Surveys.& Peer Review #SoMe4Surgery #SoMe4Error @me4_so
Every Profession has Good & Bad. It’d be naive, to believe anything else. You can try & identify the poor performers & help them improve skills,or, remove, if they can’t or won’t, otherwise the actions of a few, will reflect badly on the majority #SoMe4Surgery #SoMe4Error @me4_so
Improving: Surgery Clinical Supervision Auditing All Complication Recording Recording Nos of Patients with 1 or More Complications #RootCauseAnalysis Longterm Monitoring of All Patient Outcomes gives Better Risk vs Benefits & Complication Rates #SoMe4Error #SoMe4Surgery @me4_so
I would love hospitals to have clinical nurse specialists to support patients' emotional needs (as well as physical) after major adverse events. Where needed, they could also act as an intermediary between the patient & the surgeon. #MyIdealJob #SoMe4Surgery #SoMe4Error
