Co-locating exercise w/ cancer treatment is a promising strategy to ↑ activity for people w/ cancer. Evaluation of an existing model @GenesisCare + @EMRI_ECU demonstrated feasibility, but revealed suboptimal clinician referrals and program uptake. #ExOncTC (1/6)
6/6 Higher PA among lymphoma pts @diagnosis & in 3-yr survivors is significantly associated with OS, LSS & EFS. Increasing PA after diagnosis is significantly associated with improved OS & LSS. PA is important in lymphoma survivorship. Intervention trials are needed! #ExOncTC
9/10 I want every Dr and nurse to prescribe exercise to every cancer patient, before, during and after treatment, based on facts and not hearsay. If they weren’t sure, they’d know who to ask instead of making it up @moving_medicine #ExOncTC

4/10 The minimal exercise prescription for cancer management includes moderate intensity aerobic and/or resistance training at least 3 days/wk, for 30+ minutes, for at least 8-12 weeks. Specific benefits by mode are in Box 3. #ExOncTC
9/10 While it may be ideal for cancer survivors to have a comprehensive fitness assessment, requiring that all survivors have one prior to starting an exercise program can be an unnecessary barrier to starting exercise, especially low intensity programs. Agree? Disagree? #ExOncTC
8/10 The question of whether a cancer survivor needs a pre-exercise medical evaluation or medical clearance to exercise continues to evolve. We have adapted the NCCN Survivorship Guidelines to provide guidance around safety concerns related to cancer (Tables 4 & 5) #ExOncTC
7.10 Implementation ex 1: Peer mentoring by Reach to Recovery volunteers to deliver #PhysicalActivity counseling to breast cancer survivors was tested Pinto et al. High retention, increases in PA seen at 3 mo. #ExOncTC
6/10 Ignoring my Dr, halfway through chemo I did a pool-based sprint tri. I had started blogging and suddenly other cancer patients were asking me for advice – what could they do during treatment? #ExOncTC
While exercise during chemotherapy for breast cancer has many benefits, the optimal exercise Rx is unknown. Exercise that linearly increases in intensity & duration may fail to account for fluctuations & accumulations in chemotherapy side-effects, such as fatigue #ExOncTC
6/6 “Chemotherapy-periodized” exercise is a possible strategy to optimize exercise adherence & readiness to train in women with breast cancer by accommodating cyclical variations in side-effects. Future directions: directly comparing linear & periodized exercise #ExOncTC
5/6 Fatigue increased relative to baseline in both groups by the end of chemotherapy. Across the 3rd chemo cycle, fatigue peaked 3-5 days post-cycle 3 in both groups; although was lower for exercisers. A trend towards increased VO2peak was also seen among exercisers. #ExOncTC
3/6 We completed a pilot trial to determine if patients with cancer can exercise safely, under supervision, during chemo infusion using a randomised 🔀 design with 20 min low-intensity exercise vs usual care #ExOncTC link.springer.com/article/10.100…
6/10 The simple proposal is for clinicians to assess, advise, and refer patients to either home-based or community-based exercise or for further evaluation and intervention in outpatient rehabilitation. #ExOncTC
#ExOncTC (2/6) Exercise sessions were completed either before or after a treatment session. Two training windows, on separate days, were available for patients. An Accredited Exercise Physiologist (AEP) designed and supervised tailored exercise programs (~60 mins).
10/10 Finally, to help active cancer patients connect and motivate each other, keep training, and read the latest evidence, @lucygoss, @tanjaslater, @fullen_gill and I are creating cancerfit.me - launching soon. #ExOncTC
3/6 Exercise session attendance & aerobic intensity/duration adherence were assessed during “chemo” & “non-chemo” weeks. Fatigue was evaluated (Piper Fatigue Scale) pre & 2-3wks post-chemo & across the 3rd chemo-cycle to determine whether a cyclical pattern occurred #ExOncTC
