What I've experienced with supposed dreams of 'product first' organizations is a delusion where Product Managers still are very disconnected from development and engineering.
Without a design leader, you will be evaluated on how many pixels you push, not how well you push those pixels (sarcastic here, but lets make it meaningful)...
Lack of design leadership. The biggest concern is designers not having leaders to represent it as a core competency. What's their career path if they are evaluated on output? Spot on by many others about designers being a PM's paintbrush.
Design has been siloed in orgs, design felt this is normal or isolated and voicing up takes courage and is risky as it exposes realities on what's broken.
The execution path impacts where the design lands but it all starts with intention. Design is both forward looking and evaluated retrospectively.

The problem and challenges Design faces in orgs is lack of agency.
