To subvert expectations effectively, you first have to get the audience to care about the original expectation.
In Star Wars: The Last Jedi, Rian Johnson wanted to surprise the audience by killing off the main villain, Snoke, halfway through the film. It was a deliberate choice to subvert expectations and give Kylo Ren true dramatic autonomy.
My best memories from WoW: 1. Spontaneous 80+ Stormwind raid for the achievement 2. Hunting rare spawn TLPD and racing others in the zone to get it first 3. World change based on player accomplishments- Onyxia, etc If the world doesn't feel alive it may as well not be an MMO.
Why do you think open world zones have depth? Players tend to opt out of them and run dungeons etc as soon as they are able. Just curious

My interest has virtually disappeared upon learning this. With the right in-game reporting and blocking tools, this is a non-issue.
