Here's a chart worth remembering...authors looked at NEW ECG findings in PE (segmental up to saddle). Nice demonstration of the MYTH of sinus tach and S1Q3T3 in PE. Note-24% of patients had no new changes on the ECG

This was a tougher case than it seemed. Clues: HR varying from 120s-150s, 4gms of calcium + insulin did nothing; Hickam's dictum applies!
A 41yo is brought to the ED and is febrile, normotensive (126/78), tachycardic (120s-150s), tachypneic and POC glucose = “high”. The following ECG is obtained that shows a wide complex tachycardia. What is your differential for this wide complex tachycardia?
This is absolute garbage. Please.. show me the support of just ONE legitimate physician toxicologist who will attest to the reality of acute fentanyl OD via dermal exposure. ( it doesnt exist)
Recently, a @SDSheriff Deputy was exposed to Fentanyl and nearly died of an overdose. If it wasn't for the quick-thinking of his Field Training Officer in administering Naloxone, that deputy would not be alive today. Watch our public safety video: vimeo.com/583169640
