And remember this when they come in middle of night looking fine and parents say they were “wheezing” without asthma/RAD history 1- they did look bad (stridulous) at home 2- the cold humid air outside made better 3- it was stridor not wheezing (parents only use words they know)

In patients presenting with cUTI (including acute pyelonephritis) who are improving clinically on effective therapy, we suggest treating with a shorter course of antimicrobials, using either 5-7 days of a FQ or 7d of Non FQ rather than 10-14d 14 days for cUTI is obselete!

Infected stone is why we get the CT!! That can be life threatening and rapidly progress to sepsis and MODS.

An urgent care referred a carpenter with a splinter in his hand to me. It was removed with tweezers. Referred a laceration to the ED for sutures. Referred wrist fractures to the ED for splinting. These places offer very little value to many patients.
