I consider it fairly easy to understand that P1 Dude is a separate person from P2 - P4 Dude. P1 Dude was just some poor man whose life was going as bad as it possibly could be until he finally just snapped. P2 Dude just does as he pleases without regard to the after effects.
SO Dude initially had a breakdown when he was ~27, killed a bunch of people, put into an asylum until they figured he was rehabilitated and stable enough to support himself. They release him, he moves, gets married, becomes an addict and lives in a trailer, decides to move BACK-
