Exactly. The purpose of airway evaluation is not to ‘predict difficult intubation’ but to identify ‘at risk’ airway management.

Difficult is a retrospective description of perception of events, not a meaningful classification to assist preparation 7/

Agree that we don’t have the tools to ‘predict difficult airways’. Instead we should be trying to identify ‘at risk’ airway management, then escalating precautions as appropriate - accepting that these escalations will often prove to have been unnecessary.

We should move away from prediction of difficult airways. There is > 70 years of proof that it does not reduce complications as the current prediction is as good as the weather forecast 14 days ahead. It makes more sense to assess the airway and do an accurate risk…

As stated in the guideline, sustained exhaled CO2 excludes oesophageal intubation, it does not confirm tracheal intubation.
