Busy day, but lots of interesting cases. This one-myeloma kidney, with only rare casts on IF that were kappa light chains+, lambda-. LM few PAS neg casts. EM confirmed that casts were made of light chains as casts were made of densely packed crystals. Unusual but distinctive.
Fantastic paper🌟 they conclude that the available data do not support the use of currently available minimally invasive methods as stand-alone diagnostic tests at this point
Interesting membranous nephropathy. Early MN may be "invisible" by light microscopy - see silver stain. Segmental deposits by IF/EM associated w/NELL-1 (+) stain. Patient on lipoic acid <- likely cause (PMID: 34662650). Also underlying underlying DN. #renalpath #PathTwitter
