Poor man’s drug-eluting sinus stent. Posisep X in the right middle meatus 1) Compressed/dry, 2) Expanded/filled. I fill with a mixture of Oxymetazoline and CiproDex Otic drops. I just leave it in there forever and have the patient remove with saline irrigation. #ENTwitter

Unusual middle ear osteoma emanating from the promontory of the cochlea and causing CHL. Tricky to drill behind the malleus, and I had to shorten it by 2mm. #ENTwitter
Recurrent Respiratory Papillomatosis. Multiple surgeries over a lifetime, but doing much much better now. I use a laryngeal Coblation wand for these in lieu of laser. In my hands it allows for better finesse and tissue preservation and hemostasis. #ENTwitter
Deep lobe parotid tumors above the facial nerve are tedious to remove. Margins always tight. A tad easier if the mandible will retract out of the way easily. No real easy way to do them other than to just take time and be careful. #ENTwitter
This innocuous little thyroid mass was Bethesda Cat 5, Affirma Suspicious with + BRAF mutation. Interestingly the patient was also hyperthyroid so the TSH was significantly suppressed. I got her euthyroid on tapazole and took the thyroid, hopefully curing both. #ENTwitter

Left middle ear effusion and placement of Hubbard Airplane Tube in-office. TM is blanched from phenol. People are often surprised that we place tubes in the office. However, I place about 10x as many tubes in the office (adults) as I do in the OR (Children.) #ENTwitter
