The posteriorized embryo sac-like structures contained primordial germ cell-like cells, as well as cells that recapitulated the posterior primitive streak, resembling the onset of gastrulation. #ReproChallenge
Today’s paper asks the question: is bacterial vaginosis associated with a decreased probability of becoming pregnant? This is not ART, but natural conception in a cohort of women who are seeking a pregnancy. #ReproChallenge
The microfluidics device contained 3 parallel channels: a central channel for loading a gel matrix, a top channel for loading the stem cells, and a third channel containing flowing medium. #ReproChallenge
The anteriorized embryo sac-like structures contained epiblast-like cells at one pole and amniotic membrane-like cells at the other, resembling the human asymmetric embryonic sac prior to the onset of gastrulation. #ReproChallenge

Not much is known about BV and natural fertility. This study by @Lokker and colleagues asks: what is the association between BV and fecundability (i.e. the per-menstrual cycle probability of pregnancy) in a cohort of Kenyan women trying to conceive? #ReproChallenge
🟥Total fertilization failure: 23 cycles of conventional IVF (4.0%) and in 8 cycles of ICSI (1.4%). Low fertilization: 65 cycles of conventional IVF (11.4%) and in 32 cycles of ICSI (17.0%). More details on FR by indication 👇 #ReproChallenge
➡️Persistent BV (BV positive tests throughout the sampling) was associated with a very significant reduction in fecundity of 43%! #ReproChallenge
🟥Rate of good-quality embryos was no different between embryos from conventional IVF (16.6%±23.2%) and from ICSI (16.6%±26.6%). 🟥ET done in 510 cycles, and no difference CP, abortion, ectopic pregnancy, and CBR were not different by source of embryo. #ReproChallenge

#ReproChallenge What are your thoughts on #EggFreezing as a reproductive right?
#AcademicTwitter proving useful once again - a very relevant study for my current coursework set by Dr M Carroll. This links to my work on perturbations of calcium signalling regarding fertility - the effect of pH in the female reproductive tract. #ReproChallenge

🟠Today’s paper is a large retrospective cohort on IVF/ICSI split insemination, the practice to fertilize part of the oocytes with classical IVF and part with ICSI. Let’s see how it went #ReproChallenge ecerm.org/journal/view.p…
Right, is comparing ETs from different patients all receiving split insemination by source of fertilization technique of the embryos. They are all first fresh ET. #ReproChallenge
Let's not forget, ICSI w/o male factor does not provide more reproductive success, not in LB, not in cumulative LB, and not even in terms of less FF. #ReproChallenge thelancet.com/journals/lance…
I don’t think so but when I talk to my colleagues gynaecologists, they are really afraid of having to tell a patient that there are no embryos #ReproChallenge
🟠In our survey, 35% respondents did split insemination, mostly to avoid FF. But, with reported rates in this paper and elsewhere, approx. 30 ICSI are needed to avoid 1 FF compared to IVF. 🤔Is this worth doing 29 useless ICSI to gain 1 less FF? #ReproChallenge

It would be interesting to know if there are differences in the frequency of split IVF/ICSI cases between countries. Do variations in reimbursement policies play a role? #ReproChallenge

Embryo grading is the main determinant for which embryo to transfer. They transfer the best regardless IVF or ICSI. Yet, IVF in most cases give nice embryos #ReproChallenge
Also, @OmarRawi89 , out of curiosity, if IVF embryos are of lower morphological score than ICSI ones, do you still transfer them first? #ReproChallenge
In this study they split with 10 CCOs, how many CCOs would you need to split? #ReproChallenge
This is very interesting! It seems that the main driver, in all papers (an our survey) is avoiding FF. But the FF avoided are few, and the ICSI very many... #ReproChallenge
Very interesting topic and paper. We follow this approach in many cycles. Clinicians and clinics are scared of no ET scenarios. This is a bit assuring to them. We always transfer the IVF embryos. I’m not saying this is absolutely justified cuz of the extra cost. #ReproChallenge
What do you think? Is it worth it? Does this look like a good approach? #ReproChallenge
🛑Limitations! ➡️Retrospective study ➡️Lots of DET and TET (!) hopefully at D2-3 ➡️No data on twins and singleton pregnancies ➡️SET could have skewed the results in favor of classical IVF? ➡️High number of oocytes (not for all) #ReproChallenge
🟥FR per injected oocytes was higher (p<0.001) in the ICSI group (72.3%±24.3%) than after conventional IVF (59.2%± 25.9%) 🟥FR per retrieved oocytes was lower (p<0.001) in the ICSI group (52.1%±22.5%) than after conventional IVF (59.2%±25.9%). #ReproChallenge
➡️Embryos in ETs were: ✅166 from conventional IVF ✅66 from ICSI ✅278 from conventional IVF/ ICSI Clinical pregnancy ➡️ fetal heartbeat at week 7 #ReproChallenge
➡️Females aged 40 and older were excluded, and only cycles in which 10 or more oocytes were retrieved were analyzed in this study. ➡️Semen was always fresh. ➡️10.471 oocytes were retrieved: 4.915 were allocated to IVF and 5.556 to ICSI. #ReproChallenge
➡️555 couples underwent 571 IVF-ICSI split insemination. ➡️First cycle for 92% of them. ➡️Reason for treatment: mild mild asthenozoospermia (200 cycles), tubal factor infertility (112 cycles), or unexplained infertility (259 cycles). #ReproChallenge
Welcome to #ReproChallenge. Today’s paper covers very nice work from @KU_Leuven, where the group of @JorisVermeesch and the first author @ADestouni show that embryos with 1 or 0PN are not as bad as we thought.
In this specific cohort, which also needed PGT-M, 31% (13/42) of cycles reached ET or could repeat ET with an unaffected 0PN or 1PN embryo. The LBR per initiated cycle increased from 9.52 to 16.67%. #ReproChallenge

Like in Waddington landscape of cell fate but the heterogeneities might be resulting in changes in the “paths” it would be up to each cell’s robustness to go down the path or be derailed #ReproChallenge

😍This IMPRESSIVE work helps us understand early embryonic growth, adding complexity to the model but also raising several interesting questions in ART. What do you think?? #ReproChallenge
Finally: ➡️blastocoel pressure influences the number of cells committing to different fates ➡️blastocysts with perturbed pressure showed an incorrect number (and localization) of TE and ICM #ReproChallenge
