My personal opinion - routine use is not supported - may be situations TXA has a more pronounced benefit, but these have not yet been fully determined - After #WOMANtrial2 and another large RCT by the @NICHD_NIH we will need to revisit this lecture #SOAPAM2022 #OBAnes 🤰

Our understanding of the pathophysiology of postpartum-hemorrhage coagulopathy is limited in stark contrast to the trauma literature - e.g. Moore's three trauma fibrinolytic phenotypes #SOAPAM2022 #OBAnes 🤰

Not all bleeding is the same - TXA, will likely be ineffective in preventing or treating bleeding that is not a direct consequence of hemostatic failure - and this is not the state of most PPH at least as we know it now #SOAPAM2022 #OBAnes 🤰

Accidental spinal TXA is real problem - 21 cases of spinal TXA administration -life-threatening neurological and/or cardiac complications, requiring resuscitation and/or intensive care, occurred in 20 patients and 10 patients died @APSForg #lookalikevials #SOAPAM2022 #OBAnes 🤰

There is an abundance of low-quality clinical trials and meta-analysis that include small trials are ill informed - you cannot turn poor quality, biased trials into a composite important clinical outcome #SOAPAM2022 #OBAnes 🤰

However the extimate blood loss difference between the 2 groups was minimal at 33 mL - there was also no signifcant diffence in transfusion rates and no differnce in what the obstetricans perceived of the case #SOAPAM2022 #OBAnes 🤰

Many experts have recommended TXA be used as an adjunctive and not a compulsory medication as part of PPH treatment protocols, leaving the decision to the discretion of the care team #SOAPAM2022 #OBAnes 🤰

While the #WOMANtrial has been generally heralded by the medical community as a major breakthrough in PPH management, the limitations of the study should not be discounted #SOAPAM2022 #OBAnes 🤰

I tried to organize my review of the evidence by first evaluating TXA for the treatment of PPH, followed by the use in preventing PPH for vaginal delivery and finally preventing PPH during cesarean delivery #SOAPAM2022 #OBAnes 🤰
