As natural language processing improves, we will need less manual work from clinicians to enter data into discrete scoring tools, problem lists, etc. This means physicians will again need to write concise, helpful notes. Yay! #notebloat #NLP buff.ly/3yuxNZ8

Fascinating study in @JAMANetworkOpen examining duplicated text in clinical progress notes. I like the term "information scatter" because it nicely describes the difficulty in finding new or actionable knowledge in the patient chart. #EHR #NoteBloat buff.ly/3r8l0Yv

Yes Ritu, this is a critical advancement. Medical complexity and time will drive billing. And, you do not need a review of systems. The true time killer. #TelemedNow

#NoteBloat 🎯🎯 One of the (many) unintended consequences of the EMR.

EHRs have streamlined workflows but also introduced challenges like “note bloat.” Learn how documentation has evolved and discover strategies to reduce excess notes in our latest blog post: hubs.la/Q037lk3Z0 #ehr #medicaldocumentation #notebloat #healthcaretech

When was it decided that clinical consult/progress notes needed to be educational documents as well as medico legal documents? Can't find the actually useful stuff #notebloat

A wild study recently published by @BPattersonMD from @UWEmerMed: "The volume of notes available for review by providers has increased by over 30-fold in the 17 years since the implementation of the EHR at a large health system." pubmed.ncbi.nlm.nih.gov/38779571/

FAA starts investigation into Boeing today, looking for mechanics falsifying records where they said they did parts of the inspection that they didn't really do. Wow... the parallel to EMR templates and copy-paste notes is quite remarkable. Hmmm.... #Sjogrens #MedEd #NoteBloat

🎯 Yes... and I hope to help us clean that junk up. It's ridiculous that 30-40% of my 20-mins visit it taken up just trying to read the dang EMR notes. No.... they did not do a foot flexion/extension exam on my child who was in for an ear infection. No. Just NO. #NoteBloat

The only two likely readers of most notes are the patient and your future self. We should be courteous to both by staying relevant #notebloat

So there’s hope for some. The most likely reader(s) of your note are the patient, and your future self. Do them both a favor and stay relevant in the chart #notebloat epicresearch.org/articles/two-y…

sigh.... I guess not. Impolite? Fact is, no one reads anyone's' notes anymore. #EMR= #notebloat = scroll to the end (or not) Anyway, verbal is better, that way we know what they received #MedTwitter

“Did you read our note?” Acceptable or unacceptable for a consultant to say to a primary team? What say you, #MedTwitter?

Important study highlighting one of the drawbacks of EMR #Notebloat is a thing! #copyandpaste practice creates errors, outdated information, and increasingly expanding #progressnotes. #Templates are useful but please AVOID turning #progressnotes into #DischargeSummaries

I happened to graduate residency at a time when my H was switching to EMR. Despite obvious conveniences and potential benefits, I miss paper charts dearly. #Notebloat is a real problem, I struggle to find information wading through copy-pasted garbage
