Good point but I don't think will be in issue. In recently presented Manifest PFA registry with >17k pts stroke incidence was 0.12%. In ADVENT we did brain MRIs in subset of 80 pts and asymptomatic cerebral events noted in 9% of PFA pts, less than usually seen with RFA.
PFA's main selling point has been avoiding AEF (incidence 0.038% percent in POTTER-AF). Combined increase in embolic events and coronary spasm are surely going to nullify any safety benefit?

Our experience is that identification of high frequency zones works in reentrant atrial tachycardias!!! Look this case of localized reentry from left atrium roof. Propagation map shows centrifugal pattern of activation. High frequency matchs with the "origin" of tachycardia.

There was a prior lateral mitral isthmus line performed though, although it does not appear blocked. Would adding both anterior and roof lines in this case possibly create greatly delayed conduction to the LAA?
