Great point, and I totally agree, team-based care is needed. We still need stronger evidence supporting surgery for the primary lung tumor in these scenarios. The total number of patients receiving surgery across studies is small (n=31), and all 31 were treated in the pre-IO…
Here are some practical uses of oligo(SB)RT in the meantime 1) LCT to maintain current systemic therapy, 2) LCT for systemic therapy holiday and 3) LCT in lieu of systemic therapy.

Twice a day RT to 60 Gy is *the* standard of care for limited stage SCLC It should replace *the previous* #radonc standard of care: twice a day to 45 Gy If you don't (oftenly, persuadingly) offer twice a day RT for small cell #lungcancer ... rethink those life choices
