Salt and pepper chromatin. Nucleoli not conspicuous. Mitoses everywhere and Ki67 approaching 100%. TTF1 positive. Small cell carcinoma. No other tumour sites identified on CT/PET, so this was assumed to be a primary.

IHC stains were positive for mixed B and T-cell markers, in addition to CD30 (immunoblasts). IVL is a benign phenomenon that could be misinterpreted as lymphoma. It is reported in appendectomies and ileo-cecal resections, but could also be seen in peri-nodal tissue #Hemepath
Diagnosis: Medullary carcinoma MLH1/PMS2 expression lost. BRAF V600E mutation present. MLH1 promoter was hypermethylated (indicating sporadic MLH1 loss rather than Lynch syndrome). Key point is not to mistake it for another malignancy. x.com/DrMarkOng/stat…
