I find myself wondering why as an ED physician, I am often the one clarifying code status on cancer patients towards the end of their journey. These are important discussions and considerations that should occur upstream by their longstanding trusted treatment team.
Can we pass a federal law prohibiting use of home BP cuffs between 5PM - 7 AM? There is never any indication to check BP at home outside office hours. Any doctor who orders home BP checks for their patients should be legally obligated to see those patients immediately if ⬆️ BP
Please stop telling people with high blood pressure but no other symptoms that they MUST GO TO THE ER RIGHT NOW. When you give them this urgency, and our recommended practice is to not intervene on patients with good follow-up, we are setting everyone up to be disappointed/upset.
Ambulatory BP monitoring may be guideline-supported, but clinicians recommending home monitors MUST inform patients that there’s no such thing as a BP so high that you “have to go to the ER.” healio.com/news/cardiolog…

This is your routine #medtwitter PSA: Please call the ED when sending a patient from anywhere. It’s a #patientsafety issue. Poor communication is a top cause of medical errors every year. Don’t let it be your fault. Including nurses on triage, PCP, UC, specialists…Everyone!
Escape room for physicians where you have to… - Turn off (and on) the bed alarm - Boost a patient in bed - Pull tray table over the bed - Troubleshoot a beeping infusion pump - Silence and adjust alarm parameters on patient monitor - Disconnect tube from an IV What else?
