I think we need to look much wider than just reprimanding individuals for negligence or mistakes. Yes, undoubtedly inappropriate scope of practice and insufficient supervision contributed here, but we need to fix these at systems level. #LearnNotBlame

I represent a body that works for safety and what is ethically just. #LearnNotBlame and all that. You can spin that argument at the BMA, perhaps — a union has an interest in self-preservation — but it won't stick with us. We have direct evidence of harm and bad practice.

Good to attend @nmcnews council meeting in person 4 healthy debate. lots to do to ensure neutrality in investigations & #donoharm - #learnnotblame #justculture hoping KC investigation highlights inconsistencies in investigations and need for "attitudinal change".


One way of creating some good news stories would be to support restorative approaches to cases of WB abuse that pre-dated the new standards. Learning from historical abuse has its place in education, social care, religious institutions… why not in healthcare. #learnnotblame


We should absolutely learn from this case and I hope the Government really heeds the warnings. Nonetheless I agree, calling this "manslaughter" etc is not helpful — #LearnNotBlame is vital to healthcare safety.

But saying that they're guilty of a crime in your opinion, or that this is a "barn door" diagnosis when you've not had any detailed view or insight into this case is rather unbecoming. Perhaps reflect on that.



Historical guilt shouldn't be a birthright. Let's stop treating it that way. Our skin doesn't dictate our actions, our individual choices and cultural context do. Let's learn from history, not weaponize it! #LearnNotBlame
