I think we need to look much wider than just reprimanding individuals for negligence or mistakes. Yes, undoubtedly inappropriate scope of practice and insufficient supervision contributed here, but we need to fix these at systems level. #LearnNotBlame

One way of creating some good news stories would be to support restorative approaches to cases of WB abuse that pre-dated the new standards. Learning from historical abuse has its place in education, social care, religious institutions… why not in healthcare. #learnnotblame
We should absolutely learn from this case and I hope the Government really heeds the warnings. Nonetheless I agree, calling this "manslaughter" etc is not helpful — #LearnNotBlame is vital to healthcare safety.
But saying that they're guilty of a crime in your opinion, or that this is a "barn door" diagnosis when you've not had any detailed view or insight into this case is rather unbecoming. Perhaps reflect on that.
